Case Management
Each person served is assisted by a case manager in a client/family
directed system of coordination and accountability regarding individual
needs and preferences.
Case managers are responsible for referrals and intakes; coordinating the
development and implementation Individual Plan (IP); coordinating needed
services; monitoring and reviewing a person's progress toward individual program
goals; assisting clients and families with program transfer and termination; and
providing continuous support and guidance to the person with a developmental
disability and family.
