The 600 consumers served by MDS participate in one or more of the following programs. Diverse, individualized services and supports are provided to meet the needs of
each person. All programs stress personal growth, independence, freedom of choice and integration in the community. |

Case Management Each person served is assisted by a case manager in a client/family directed system of coordination and accountability regarding individual needs and preferences. |

Case managers are responsible for referrals and intakes; coordinating the development and implementation of the Individual Plan (IP); coordinating needed services; monitoring and reviewing a person's progress toward individual program goals; assisting clients and families with program transfer and termination; and providing continuing support and guidance to the person with a developmental disability and/or the family. |

Infant/Toddler Early Intervention Services Service coordination, individual occupational, physical and speech therapies, and parent education and training are provided as needed in the family home, a unique aspect of the program. Parent participation is important. |

Family Support Services This program provides customized supports and services to enable families to keep their child with a disability in the family home. Examples of assistance are: home modifications, vehicle modifications, adaptive equipment, durable medical goods, therapy, respite care, home health care services, etc. |

Children's Extensive Support Waiver

The Children's Extensive Support Waiver (CESW) helps families meet the challenges of caring for a severely disabled child by providing Medicaid benefits and additional targeted services and supports to those children most in need.
